Job Description

CloseKnit is seeking a Non-Clinical Health Guide (HG) to join our care team. We are a virtual first advanced primary care practice committed to providing high-quality, team-based care that integrates seamlessly with our patients’ lives. Our model prioritizes clinical excellence, patient engagement, and innovative care delivery. In addition to primary care, we lead a national advocacy program that helps patients navigate the healthcare system and access the care they need.

Role Overview

The Non-Clinical Health Guide (HG) plays a critical role in delivering patient-centered care across our virtual primary care practice and advocacy program. Their primary responsibility will be to serve as the main point of contact for patients and their families. The HG will work closely with our clinical team to provide personalized guidance, facilitate access to resources, and enhance patient engagement and satisfaction.

Key Responsibilities
• Patient navigation and engagement:
• Act as the primary contact for patients, assisting them in navigating their care, scheduling appointments, managing referrals, and following their treatment plans.
• Build and maintain strong relationships with patients and their families, encouraging active participation in their care plans and adherence to treatment recommendations.
• Facilitate clear and timely communication between patients and health care providers, ensuring all patient concerns and questions are addressed proactively and promptly.

Conduct regular proactive outreach using CloseKnit-established workflows to monitor patients’ health status, address any issues, and provide ongoing support.
• Resource coordination and patient education:
• Identify and connect patients with appropriate healthcare resources, community services, support programs, and employer programs.
• Provide patients with educational materials and support to help them understand their conditions, treatment options, and preventive care measures.
• Provide patients with timely information on their health care benefits and provide timely insights into all financial and billing questions patients may have about services rendered by their primary care team.
• Collaboration & team integration:
• Work closely with a multidisciplinary team of providers, health guides, and medical assistants to create a seamless patient experience.
• Collaborate with the care team to design new team-based workflows as needed. Participate in quality improvement initiatives to enhance patient care and service delivery.

Qualifications

Required
• High school diploma with at least 7 years of experience in health care environment, customer support health coaching, or community health navigation.
• Proficiency in Microsoft Office use
• Proficiency in working with electronic medical record systems.
• Proficiency in working with technology and across multiple systems.
