Provide personalized support and guidance to members throughout their healthcare journey.
Utilize strong analytical skills to identify and solve complex problems.
Maintain a deep understanding of Aetna’s policies and procedures related to case management.
Collaborate with other team members and departments to ensure efficient and effective service delivery.
Conduct thorough assessments of member needs and develop individualized care plans.
Communicate with members, healthcare providers, and other stakeholders to coordinate care and address any concerns.
Monitor and track member progress and make necessary adjustments to care plans.
Stay up-to-date on industry trends and best practices in case management.
Uphold Aetna’s standards of confidentiality and data protection.
Provide excellent customer service to members and ensure their satisfaction with Aetna’s services.
Meet performance metrics and goals set by Aetna.
Act as a liaison between members and Aetna to facilitate effective communication and resolution of issues.
Continuously evaluate and improve processes to enhance the member experience.
Adhere to all regulatory and compliance requirements.
Continuously develop and expand knowledge and skills through training and professional development opportunities.
